We recommend pre-registration for all our new patients. This allows us to verify all demographic and insurance information prior to your visit, so that all of your paperwork is completed, thus decreasing your wait time when you arrive for your visit.

Please remember that all co-payment, deductibles and any other patient responsibilities are due prior to or at the time of service.

Pre-Registration will benefit you by:

●  Allowing us to verify benefits and eligibility with your insurance plan, and resolve any concerns before your appointment.
●  Informing you of financial obligations beforehand so you can plan for the unexpected.
●  Reducing checkout delays by having this information prior to your appointment.

We offer two convenient options for pre-registration:

●  Pre-Registration Forms are available on our website or at any of our clinics during regular business hours.
●  Mail or fax all forms with a copy of your insurance card (front and back) and driver's license or you can drop them off at clinic.

After your initial appointment you will need to arrive at least 15 minutes prior to any established appointment to update any necessary paper work and collect any co-pay, deductible or coinsurance.
